Insurance Participation

We are in-network with many plans, and contracted through a third party for most others. If we are contracted through a third party your insurance companies website may not have us listed as participating even though you can be treated at our office and your insurance will cover the allowable fees based on your policy.  If you would like to find out if we work with your insurance the easiest way is to take a picture of your insurance card, both front and back, and text it to us at 801-254-0713 and we will get back to you.
